Follow- Up Supports

Homes First operates as a 4-person Follow Up Support Worker team that are matched with individuals who have been recently housed from the shelter system (including non-Homes First shelters). The role of each worker, who each carries their own caseload, is to help individuals maintain their housing and connect to resources and services in the community. They meet individually with each client (or as part of a team consult) to help each client move towards and maintain stable housing. Their role includes case management planning, crisis intervention, supportive counselling and advocating for and with the client with landlords, service providers and income assistance programs.
