This is a multi-service collaborative funded by the City of Toronto. The team provides immediate de-escalation and crisis intervention response, telephone supports, visits in the community, and short-term follow up supports to vulnerable and marginalized individuals in the community. Six agencies in Toronto have come together to build this team: Dixon Hall, Gerstein Crisis Centre, Homes First, Inner City Family Health Team (ICFHT), Seeds of Hope and The Neighbourhood Group. The team operates in strict catchment areas in the downtown east of Toronto – Jarvis to Parliament, Carleton to Queen St. East, and Yonge St. To Jarvis St. And Bloor Street East to Carleton St.
