The Here to Help (H2H) program is a multi-service program funded by the City of Toronto.
The H2H team provides immediate de-escalation and crisis intervention response, telephone support, visits in the community, and short-term follow up supports to vulnerable and marginalized individuals in the community.
The H2H team is formed by five agencies across Toronto: Dixon Hall, Gerstein Crisis Centre, Homes First, Inner City Family Health Team (ICFHT) and Seeds of Hope. The H2H Team operates in the catchment areas in the downtown east of Toronto – Jarvis to Parliament, Carleton to Queen St. East, Yonge St. to Jarvis St and Bloor St East to Carleton St.
